Permission & Release
By signing below, I give permission for my child to attend Falls Creek Youth Camp with Dale Baptist Church. I understand that camp activities may include swimming, outdoor sports, and other physical activities. I authorize camp staff to seek emergency medical treatment for my child if necessary and I cannot be reached. I have reviewed the camp rules with my child and agree they will comply.

I also give permission for my child to ride with an adult sponsor of Dale Baptist Church for any and all transportation during the Falls Creek Youth Camp week, including to and from camp and any other necessary activities.

I further give permission for my child's image, likeness, and/or voice to be used in any photos, videos, or promotional materials produced by Dale Baptist Church.
